I would suggest changing the Name to UltraStar Deluxe Portable and the AppId to UltraStarDeluxePortable to be consistent with the app's real name.

You need to have appicon_16.png and appicon_32.png (and optionally appicon_128.png) alongside your appicon.ico.

and remove the licensing line about freeware since this is open source.

Still just the template, you need to fill in the app name, description, links etc.
